This is a FREE 6-week program for adults of all ages who have Diabetes, Heart Disease, Arthritis, Chronic Lung Disease, Asthma, Fibromyalgia, and other long term illnesses. This peer-led group meets for 2.5 hours a week over 6 weeks.

A Healthy Life & Chronic Conditions

From the Living a Healthy Life with Chronic Conditions book that you will receive on day 1 of the series and learn from throughout the six weeks:

Nobody wants to have a chronic long-term illness. Unfortunately, most of us will experience two or more of these conditions during our lives. This book has been written to help people with chronic illness explore healthy ways to live with a physical or mental condition. This may seem like a strange concept. How can you have an illness and life a healthy life? To answer this, we need to look at what happens with most chronic health problems. These diseases, be they heart disease, diabetes, depression, liver disease, bipolar disorder, emphysema, or any one of a host of others, cause most people to experience fatigue as well as to lost physical strength and endurance. In addition, they may cause emotional distress, such as frustration, anger, anxiety, or a sense of helplessness. Health is soundness of body and mind, and a healthy life is one that seeks that soundness. Therefore, a healthy way to live with a chronic illness is to work at overcoming the physical, mental, and emotional problems cause by the condition. The challenge is to learn how to function at your best regardless of the difficulties life presents. The goal is to achieve the things you want to do and to get pleasure from life. That is what this book [and workshop series] is all about.

What You Will Learn

This workshop series runs for 6 weeks and each week has a direct focus.

Week 1 – Managing symptoms with your mind and action plans

Week 2 – Dealing with emotions and physical activity

Week 3 – Breathing techniques, pain management, and muscle relaxation

Week 4 – Healthy eating, communication, and problem-solving

Week 5 – Medication usage, treatment decisions, and depression management

Week 6 – Working with your healthcare professional, and planning for the future

The book you will receive as a participant has additional information on:

  • Becoming an active self-manager
  • Finding resources
  • Understanding and managing common symptoms
  • Using your mind to manage symptoms
  • Exercise and physical activity for every body
  • Exercising for flexibility, strength, and balance: making life easier
  • Exercising for endurance: aerobic activities
  • Communicating with family, friends, and health care professionals
  • Sex and intimacy
  • Healthy eating and healthy weight management
  • Managing your medicines and managing your treatment decisions
  • Managing specific chronic conditions
  • Planning for the future: fears and reality

Peer Leaders

The aim is to have the workshop series led by peers who have similar chronic conditions, have participated in the series, and who have benefited from the program.
