This is a FREE 6-week program for adults of all ages who live with Chronic Conditions. This could include but is not limited to diabetes, heart disease, osteoporosis, COPD, Cancer, hypertension as well as many other chronic health issues.
This peer-led group meets for 2.5 hours a week over 6 weeks. Each week individuals learn skills that can help them better manage their symptoms as well as their daily lives. Many topics are covered including fatigue management, mind-body connection, action plans, problem-solving, dealing with difficult emotions, physical activity and preventing falls, making decisions, health eating, pain management, communication skills, dealing with depression, self-talk, working with your healthcare team, planning for the future and more.
Participants in this workshop will receive a helpful chronic conditions Self-Management Program resource guide book.
Family members and caregivers are also welcome. Anyone can register for this workshop.
