Support for People with Health Conditions
The North Simcoe Muskoka Self-Management Program offers free self-management workshops to help empower people to take charge of their health, while living with health conditions like: diabetes, heart disease, arthritis, lung disease, and other chronic health issues.
Living a Healthy Life Self-Management Workshops are:
- Free six-week workshops for adult patients and caregivers
- Developed by Stanford University, and are a Stanford Chronic Disease Self-Management Program
- 2.5 hours each week in groups of 10-16 participants
- Led by trained volunteer facilitators living with one or more chronic conditions
- Offered in the following languages: English, French
Who Can Attend?
All our programs and services are for individuals older than the age of 18 who are living with a long-term health condition.
Anyone can register themselves for a workshop; no referral from a health care professional is needed.
Who leads the program?
These programs are led by individuals who have experience living with various long term health conditions and have completed the evidence-based training material. These individuals have been selected and trained to offer a variety of training within the NSM region.
Is there a cost to attend?
There is no cost to attend.
